Look for TPE in patients presenting with Hypereosinophilia.

NCCT Thorax shows innumerable tiny nodules of ground glass attenuation scattered throughout both the lungs.
Tropical pulmonary eosinophilia is one of the extrinsic causes of Pulmonary Eosinophilic Syndromes.

The onset of tropical pulmonary eosinophilia is slow, and the major symptoms are fever, nocturnal cough, dyspnea, and wheezing.
Note: Many patients present with asymptomatic state ans incidentaly detected hypereosinophilia (as this patient in clinical scenario)

This patient was given DEC 100 mg TDS
On day 4 he came back with dry cough and mild wheeze.
Prednisolone 30 mg OD + Monteleucast 10 OD and L-cetirizine 5mg OD (Conilsidering airway reactivity)
Symptoms subsided and it was stopped on day 15.
